refactor: rename lanhu-context-cli package to @lanhu-context/cli, repo to lanhu-context

Unscoped publish was rejected (E403) by the org-scoped npm token; moving the
CLI under the @lanhu-context scope matches the token grant and the renamed
GitHub repo. Bin names (lanhu / lanhu-context) unchanged.
