Persist resolver-supplied review invariant keys across heads.

pr-snapshot mark --invariant-key records unique heads per key. Trajectory emits invariant_rounds (counts only — never inferred from comment text). A third round for the same key is needs-human before another fix.

Fixes #1575

Rounds are recorded only after a fix completes, so a pass starting at
rounds == 2 is the third attempt for that key. Every trigger was written
>= 3, which let the third fix run (and possibly clear the feedback so
the escalation never fired). State the condition as >= 2 for a key the
pass would continue, merge pipeline-mode's two overlapping sentences,
and assert the two-round trigger state in the snapshot test.

Solid work overall — the fact/judgment split landed where it should (the leaf returns the invariant_key, babysit persists it at the atomic mark boundary, the script counts unique heads and never infers keys from content), and both earlier Codex P1s were genuinely addressed.

One real bug remained, and I pushed 031e4a1 to fix it: an off-by-one in the trigger. mark records the key only after a fix completes, so a pass that starts with rounds == 2 IS the third fix attempt — but every trigger was written >= 3, so the third same-key fix would run anyway (and could clear the feedback so the escalation never fired). That contradicts both #1575's expected behavior and full-mode's existing "3rd pass would begin: stop looping" contract. All five spots now say >= 2 for a key the pass would continue, pipeline-mode's two overlapping sentences got merged into one condition, and the snapshot test now asserts the two-round trigger state at the boundary, not just that three marks yield three rounds.

Full babysit test files pass locally. Merging once CI agrees.

An answered two-round escalation left rounds at 2, so the gate re-raised
the same needs-human and the persistence layer rejected the re-park:
gate now fires only while the key's escalation is unanswered, and an
answer authorizes the next action. The leaf contract also implied one
invariant_key per pass; it is one per fixed root, persisted on each
item's dispatched mark, so unrelated roots accumulate rounds separately.

Second edit round against this block; fold trigger, escalation, the
answered-escalation exception, and the anti-cry-wolf line into one
escalate / execute-answer / fix-as-usual decision instead of stacking
qualifiers.
