[Portal] Link to the help desk with known details prefilled - #5990

Replace the go.authgear.com/portal-support link in the project limit and SMS/WhatsApp country notices with the help desk form, passing the project ID, email, name and purpose when known.
The short link forwards the query string, so the help desk target stays changeable without a portal release.

The project limit notice (Projects page) and the SMS/WhatsApp countries notice (Login methods → Phone) linked to
go.authgear.com/portal-supportwith no details.
