feat(observability): instrument event and websocket handlers with OTEL

Description

Wire OpenTelemetry counters into event publish paths and WebSocket lifecycle.

• Count accepted/rejected events via createEventCommandResult (NIP-20 OK unchanged)
• Track active WebSocket connections on connect/close
• Builds on feat(observability): add OpenTelemetry metrics bootstrap with Prometheus sidecar #653 OTEL bootstrap; no admin API or UI yet
