feat: remove supervisor mode - #81

Completely remove supervisor functionality (Agent stop-time work review), replaced by Claude Code's built-in goal and loop features. - Remove internal/supervisor/ and internal/llmparser/ packages - Remove supervisor CLI commands (supervisor-hook, supervisor-mode) - Replace EnsureStopHook with RemoveStopHook to clean up leftover hooks - Remove supervisor config fields from Config struct - Clean up supervisor slash commands, state files, and log files on provider switch - Remove go dependencies: uuid, claude-agent-sdk-go, jsonrepair, gojsonschema - Rebrand project as "Claude Code Configuration Switcher" - Update README, CHANGELOG to reflect this change
